Co-Op Profit Sharing Program
Terms & Conditions:
Six months from the day your sign up form is processed - you and/or your immediate family will have private air-medical jet service available, free of charge, should you need it.
All benefits of participation in the Golden Keep Foundation's program are subject to current and consecutive monthly payments. If a lump-sum annual payment was made, an additional payment must be received by the same sign-up date of the following year, and each year thereafter, for continuation in the program.
Participant and/or their immediate family members may request aircraft usage only for very serious illnesses and/or injuries. These situations will be coordinated and evaluated by, and at the discretion of, a medical doctor(s) working with our affiliate - Air- Medical Systems of America.
Participant understands that in the event air-medical jet transportation is necessary, medical staff working with our affiliate - Air-Medical Systems of America and the J. Zackery Foundation, will make the determination as to which aircraft will be used.
When you sign up for your Golden Keep Card at a $10.50 per month payment, youâll be entitled to the following benefits:
When you sign up for your Golden Keep Card at a $15.50 per month payment, youâll be entitled to the following benefits:
- Private air-medical jet transportation for you and/or your immediate family members in the event of serious illness or injury.
- Automatic participation in the Golden Keep Foundation's Co-Op Profit Sharing Program to the extent that you will receive a monthly check from us in the amount of $.50 to $25.00 and one free product of your choice from our product list on the Golden Keep Card Sign Up Form.
- The satisfaction of knowing that you are contributing to AMSAâs efforts to expand air-medical coverage across America.
- A complimentary copy of AMSAâs quarterly newsletter.
Product Terms & Conditions:
All products may be returned within thirty (30) days from the delivery date for a 100% refund. The refund amount will be approximately one-third of your monthly payment or as governed by applicable state law.
Products will be sent to the address indicated on the participant's sign-up form. The participant may change the shipping address along with product type at any time, you can change this information by filling out the packing list that comes inside each order - all changes must be made in writing.
